pl contact yr nearest unit trust of india pan service centre and apply for the duplicate pan card along with xerox copy of the old pan card with proof of address and id proof

very simple,do not worry.it is simple question of Rs.67/=and apply for fresh duplicate pan with all proof and zerox of previous card.
