Your employer for 2004 is the best place to start for a copy of the old W-2. If they don't have a copy they still likely have the $ figures in their records. If they don't have the form and you need/want more than the '04 figures, you can also acquire a copy of your 2004 return from the IRS, if you filed it with that W-2, for a fee (and possibly also from your state dept of revenue, the state fee may be less than the federal fee or may be nothing). You can also get a free tax return transcript from the IRS -- not the actual forms but financial data from them -- by calling 1-800-829-1040. In the unlikely event that you just want a blank W-2 for some reason, you can down load one at no charge from irs.gov.

Attempt to contact the person or business who did your taxes that year or employer.
If that isn't possible, you can get a copy of a previously filed tax return (last 7 years) from the IRS by filing form 4506. It will cost you $39 per return that you request. Here's the link to the form: http://www.irs.gov/pub/irs-pdf/f4506.pdf |

go to the person who did your takes in 2004. They are required by law to hold onto your tax return copy and any w2s you gave them for 5 years. Just call them and they can look for you, they may charge a small fee however to reprint them. |
