I read that I am required to pay tax and a 10% penalty fine to the IRS for premature 401k withdrawal.?
On what income do I have to pay tax? Only that year's (in which I withdrew) or entire period during which I kept my money in the 401 account? Also the 10% penalty is on what amount exactly?

It has nothing to do with your income. You pay the taxes and penalty on the amount you withdrew.

If you've already withdrawn money from your 401K and it wasn't a loan that you are repaying, you need to get some face-to-face professional advice as it's complex. There are situations (hardships) in which you can take early withdrawals. The penalty and the tax is on the amount you withdraw. You did not pay taxes when putting the money in the account, so you pay when you withdraw. The penalty is to dissuade people from taking such withdrawals.

The tax and penalty are both based on the amount you withdraw from the 401k.

Tax and penalty are due on the entire amount withdrawn.

There are a few exceptions to the 10% penalty but, chances are, you don't qualify for one of the exceptions (unless you are dead or disabled).

You'd pay income tax for the year you withdrew money from your 401K, and just on the amount you withdrew. If you are under age 59-1/2, you'll also pay a 10% penalty on the amount withdrawn.

when i withdrew my 401k last year they took taxes out immediately then gave it to me. then i got a 10% penalty on the total amount that I withdrew and had to put that on my taxes this year ( they sent me a form with the amount)

When you withdraw money prematurely from your 401K, they will take out 10%. Then, on your tax return, you will pay another 10%....of the amount(s) that you actually withdraw. If you roll it over into an IRA, you won't be taxed at all (Until you withdraw it). Or, if you could qualify for one of the "exceptions, 1st time home buyer, education expenses, etc, then roll it over to an IRA first, then take the withdrawal, and you can fill out a 5329 to avoid additional taxes on the withdrawel amount.

The 10% penalty is imposed on the amount withdrawn and taxed. If your balance in the 401(k) was 25,000 and you withdrew $8000, your penalty would be $800.
