A large certified envelope suggests that you have been audited--and you would know about it--and there is two copies of a large audit report and Notice of Deficiency that is your ticket to Tax Court OR You have a delinquent account and are getting a Notice of Intent to Levy. In either case, not picking up the mail does not help you.

My guess, and it is only a guess, is either a final notice of intent to levy or notice of federal tax lien.

It could also be an appointment letter, a notice of assessment of trust fund taxes, any other letter to confirm address, and I am probably missing a few possibilities.

Pick it up and see. The IRS is required to send these notices certified to the last known address; they have no responsibility whether you pick them up, ignore them, refuse them, or have moved, left no forwarding address. You at least should want to know what is coming down the line for you.

Legally you can not refuse service. Pick up the envelope. It might not be so serious.

I you mailed paper return, IRS may have found some serious error. Did you sign your return?

Since it is certified then its important and you should reply to it immediately. IRS sends certified letters when the letters previously sent were ignored.
It could Letter 3219C if you have not filed all your tax returns. The letter gives you 90 days to file or IRS will file for you.

Pick the letter up at the PO and read it and if you have any questions contact the phone number listed on the letter.

It is my guess that the IRS has made changes to one of your returns or the IRS has filed a return for you if you have not filed and there is a balance due.
