Both, with an AND in between so you both have to sign it.

EDITED: I just thought of this. It depends on your state. Wisconsin is a 'marital state'. This means that when you are married EVERYTHING is 50/50. Anything you own, your house, your loans and your debt. Even if you had a credit card before you were married, once you are marred it is both of your responsibility. SO almost everyone is right. If your state is not a marital state it will be made out to head of household.

Each year our check is made out to my husband and myself so the answer to your question is both. It does not get made out to head of household only. If you file a joint return, both names will be on refund check.

I work as a tax preparer, so whoever is listed as the head of the household or the primary filer is whose name the checks will be in.
